Mom and Dad Promo Pass Policy
Terms and Conditions
Refund Policy. Payments are non refundable and there are no refund for partially used class pass.
Pass Duration. Mom and Dad Promo Pass is valid for 2 months from the date of purchase.
Shareable Policy. Mom and Dad Promo Pass is NOT transferable to another individual.
Freeze and cancellation Policy. There will be NO freezing or cancellation allow for Mom and Dad Promo Pass. .
Class Late Cancellation/No-Show Policies. Class cancellation must be made 12 hours prior to class start time. Cancellation must be made through our online booking system. For late cancellation and No-Shows will result in class deducted from your class pass for the scheduled session.
Behaviour. At HerbTree Studio we urge you to respect yourself and others. We do not tolerate abusive or threatening behaviour and language. We reserve the right to ban any persons exhibiting such behaviour. If you breach our behaviour policy you will be asked to leave the premises immediately. In this instance, no refunds will be given.
Change of T&Cs. We may change our terms and conditions and policies from time to time, so please check this page regularly.
Waiver of Liability
I hereby acknowledge that Pivot Redirect Sdn. Bhd. (The “Company”), group classes and/or all other activities held at any HerbTree studio location (hereinafter jointly known as “Programs”) are designed to provide general fitness and/or aromatherapy exercise guidelines by a group or personal fitness (hereinafter “Instructor”). I understand there are health risks associated with activities in these Programs. The health risks include, but are not limited to, transient dizziness, lightheaded, fainting, nausea, muscle cramping, musculoskeletal injury, joint pains, sprains and strains, heart attack, stroke, or sudden death. If I experience any of these or any other symptoms while exercising, I will discontinue the activity, notify the Instructor, and consult my physician. I am capable of performing physical exercise and acknowledge that I am voluntarily participating in these Programs. I am participating in the Programs with knowledge of the dangers and risks involved. I understand that I am fully responsible for complying with any restrictions prescribed for me by my personal physician and that I agree to consult my personal physician for further evaluation and such medical care as I require. I acknowledge my participation in these Programs is at my sole risk. I have been advised to consult with my personal physician before participation in the Programs. If recommended by my physician, I will consult with him/her on a regular basis. The Instructor and Pivot Redirect Sdn. Bhd. employees & owners (hereinafter “Team Members”) are not responsible for monitoring my compliance with my physician's recommendations. Even consultation with my regular physician is in no way a guarantee against the possibility of adverse occurrences during these Programs.
Aromatherapy, Nutrition & Dietary Information
Instructors and Pivot Redirect Sdn. Bhd. Team Members are NOT licensed
nutritionists; therefore, I understand, any and all statements made by
either of them are solely their personal belief, practice, and/or
opinion. I will not take or construe their statements as medical or
professional advice. If I have any questions about my health,
nutrition or diet, then I will ask my physician or seek a licensed
nutritionist or therapist for their professional recommendation.
Personal Property
Pivot Redirect Sdn. Bhd. is
not responsible for any lost or stolen items. Personal property or
valuables left anywhere on Pivot Redirect Sdn. Bhd. premises are at my
own risk and is never under the care or watch of a Pivot Redirect Sdn.
Bhd. Team Member or Instructor.
Medical Emergencies
I fully understand that the
staff and contractors of Pivot Redirect Sdn. Bhd., are not physicians
or medical practitioners of any kind. With that in mind, I hereby
release Pivot Redirect Sdn. Bhd. to render first aid to myself in the
event of any injury or illness. I also release Pivot Redirect Sdn.
Bhd. to call an ambulance if they are unable to contact myself, the
guardian or my nominated next of kin, or if they deem it otherwise
necessary due to the severity of the accident. I also hereby agree to
pay for such ambulance. As a parent or legal guardian, I agree to
provide health insurance for the minor child and/or guarantee payment
of any medical expenses incurred as a result of training, performing,
or participation in activities with Pivot Redirect Sdn. Bhd. I hereby
agree to inform Pivot Redirect Sdn Bhd of any medical conditions
(mental or physical) or medications they should be aware of, including
but not limited to breathing problems, seizures, allergies, Downs
Syndrome, dizzy spells, previous neck or spine injuries or conditions,
broken bones, high blood pressure, diabetes, autism, epilepsy, heart
condition etc.
Release of Liability
In consideration for my
participation in these Programs I, my family, heirs, executors,
representatives, administrators, waive, release, and forever discharge
Pivot Redirect Sdn. Bhd.’s respective owners, shareholders, employees,
and Instructors, from any and all responsibilities, liabilities and
lawsuits, present or future, and causes of action for ordinary
negligence, whether foreseeable or unforeseeable, arising out of or
related in any manner directly or indirectly, to my use of or access to
the Pivot Redirect Sdn. Bhd.’s premises and my participation in their
Programs.
This waiver includes, but is not limited to such claims that may result from any injury, illness, or death, accidental or otherwise, during or arising in any way from my participation in any exercise or recreation activity or fitness testing associated or workshop with the Programs. I hereby agree to expressly assume and accept sole responsibility for the risk of injury or death so long as they are not the result of gross negligence by by Pivot Redirect Sdn. Bhd. and/or my Instructor.
